We provide culturally-affirming, trauma-informed support to individuals with mental health disorders to access resources, achieve independence & recovery. Case Management How We Help Our culturally-affirming, trauma-informed case management programs support individuals with mental health disorders in accessing resources and achieving independence and recovery. Levels of Support Recovery and independence are different for every individual. Rooted in the belief that mental health should not stand in the way of reaching your goals, our case managers will work with you to create meaningful, achievable goals and help you reach them. At every level of support, common services include traditional case management, connections to resources, systems navigation, medication management, and therapy. Assertive Community Treatment (ACT) ACT is an evidence-based model for high-intensity, wrap-around case management. Community Support Team Community Support Team provides high-intensity, case management. Community Connections Community Connections focuses on building supports and independence. "The services I receive are excellent and help me reach a sense of independence. They have impacted me in several ways, mainly with my mental stability and a sense of how incredible life can be. I'm not afraid to ask questions, and I always get the help I need.” - Community Support Team Participant Eligibility & Enrollment Our case management programs serve Medicaid-eligible individuals in need of mental health services. Please note, while we may connect participants to housing resources, this is not a housing program. These programs serve individuals with mental health needs beyond housing alone. Colbert & Williams Comprehensive Class Member Transition Program (CCMTP) Association House is one of thirteen agencies in Illinois proudly helping individuals transition out of nursing facilities to the least restrictive level of care in community-based settings through intensive case management. I oversee community health education and intervention initiatives, as well as workforce development and technology training programs. I have over seven years of experience developing and implementing programs to assist underserved, racial and ethnic minority, and at-risk urban populations. I also hold a Bachelor’s in Psychology from Loyola University Chicago and a Master of Science in Nonprofit Management from Spertus Institute. What about your work inspires you? I strongly believe that Association House continues in the settlement house tradition of working toward a healthier community. I am proud and inspired to work with a dedicated team, who work to empower and elevate residents in areas of education, mental health, and employment. What's a fun fact about yourself? I am a life-long learner. During the pandemic, I ran with my passion for skin care and became a licensed esthetician. We offer a variety of health education services, cost-free to participants, to address critical community health needs. Check out our workshop series today! Community Health Our Community Health programs address many of the chronic community health issues facing low-income populations in underserved Chicago neighborhoods. Surveys have found that prevalent issues include food insecurity, obesity, sexually transmitted infections, diabetes, heart disease, and mental health issues. We offer a variety of health education services, cost-free to participants, to address these critical community health needs.
