Community Health Our Community Health programs address many of the chronic community health issues facing low-income populations in underserved Chicago neighborhoods. Surveys have found that prevalent issues include food insecurity, obesity, sexually transmitted infections, diabetes, heart disease, and mental health issues. The Know Your Status program provides culturally-competent, client-centered and evidence-based HIV Prevention, Intervention & Education programming at no cost. Know Your Status How We Help Association House of Chicago’s Know Your Status Program utilizes a mobile unit, its main location and partner locations as sites to provide culturally-competent, client-centered and evidence-based HIV Prevention, Intervention and Education programming throughout Chicagoland. The program’s goal is to reduce incidences of infection, help illuminate undiagnosed cases, and raise general awareness about the infection. Our services are available to anybody at no cost. Where to Find Us We provide community outreach services and educational workshops at drug/alcohol rehabilitation programs, recovery homes, health fairs, alternative schools, social services agencies and faith based organizations within the city. Request a Test/Appointment We provide rapid HIV testing using INSTI HIV-1/HIV-2 Antibody tests. Accurate results are provided in 1 minute. Or test yourself at home. We provide OraQuick rapid HIV tests by mail. This easy-to-use in-home test provides accurate results in 20 minutes. After the Results We have made it possible to work and connect with other HIV/AIDS medical providers within the city in order to provide safe and accesible medical care. We want people to know they will continue to be supported, no matter what their results may be. We're Here For You We are here to support you in more ways than one. We offer referrals for housing services, assistance with public benefits applications, mental health counseling, substance abuse services, navigating the Affordable Care Act, and so much more. We provide intensive services to keep families together and safe. Our bilingual case workers help families build a plan to stabilize themselves over time. Intact Family Services How We Help Intact Family Services provides intensive services to keep families together and children safe. With the support of a case worker, families build a strength-based plan to stabilize their family over the course of six months. Our Approach We believe that every family is capable of positive change and believe in the right of every family to receive services that are non-judgmental, respectful, and professional. Our caseworkers work alongside our families to collaboratively find solutions that work. We respect all cultures and have one of the largest bilingual English/Spanish teams in Illinois. "The Intact program helped me so much with understanding a lot with my kids and helping with myself. My caseworker was the best. She helped me so much, always checked up on my kids and myself, and even helped my grandma. She always had an amazing vibe when she came by. We really appreciate her so much." - Program Participant Referrals All referrals for this program come from the Department of Children and Family Services. If you suspect a child is being abused or neglected, please call 800-25-ABUSE (800-252-2873).
