SOAR Case Manager / Benefit Coordinator

About the Role
Responsible for assisting class members residing in SMHRFs (Specialized Mental Health Rehabilitation Facilities) and NFs (Nursing Facilities) with applying for Social Security Benefits through the SOAR program (SSI/SSDI, Outreach, Access, and Recovery). Perform Assessment to determine eligibility gathering supporting documentation, completing, and submitting the necessary forms, and assisting class members in appealing social security admiration decision, if necessary. Uses intensive outreach strategies to engage participants in services. Makes sure that class members are safe and obtain timely access to services. Serves as a liaison with other systems of care, including SMHRFs staff and collaborating treatment provider. Documents all services using program standards.
Requirements
Bachelor’s degree from an accredited college or university in psychology, social work, or a related field (or)
Certification of Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS)
SOAR certification, if not certified, must successfully complete SOAR training through the SAMHSA National SOAR TA Center within 40 days after being hired.
Experience working within the field of mental health, substance use treatment, or I/DD services preferred.
Experience working with diverse ethnic populations.
Personal vehicle, Illinois current driver’s license and insurance, good driving record acceptable to agency insurance carrier.
Ability to occasionally lift to approximately 35 pounds when applicable for transporting wheelchairs.
