Outreach Worker – CCMTP

About the Role

Provides Outreach services to Colbert and Williams Class members residing in Nursing Facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs). Provides information regarding their rights to seek services and supports to integrate back into a community-based setting under the Colbert and Williams Consent Decrees. Obtains records for CCMTP transition activities. Develops diplomatic communication and relationships with assigned facilities. Documents all Outreach activities in internal/external databases.

Requirements

  • High School diploma + 1 year of experience with the service population.  

  • Computer skills required, data entry, Microsoft office programs. 

  • Experience working with multi-ethnic populations preferred. 

  • Experience working with persons with psychiatric disorders, substance use disorders, I/DD, and complex medical conditions preferred.  

  • Excellent Communication skills and ability to connect with wide range of individuals. 

  • Must have daily use of a car, current insurance, valid driver’s license, and a driving record acceptable to Agency insurance carrier.
